Here is an example of a high quality tool for installing threaded 
inserts and an example of a properly set insert.  If you don't have and 
don't wish to buy the fancy tool, I have set them by threading a grade 8
nut onto a grade 8 bolt then a washer then the insert.  Insert the 
assembly into a drilled hole of proper size, hold the bolt with one 
wrench (Allan heads are nice here) and tighten the nut with another 
wrench until the insert is properly installed.
