I've posted a few pictures of my Index model 40 mill.  On the 
shots of the upper part notice the quill power feed which I hear 
is a rare option and the power feed on the table, which is another 
option.  

In photo 3 I have my finger on the manual kick out for the quill 
feed.  The flag just above is the auto kickout.  The engage is the 
lever just to the left on the front.  

Photo 4 shows the table feed and engage lever.  The triangle piece 
just above the engage lever is the manual kickout and the rod across 
the front is the auto-kickout.  The feed distance is set by the 2 
collars on the rod.  Also showing in this picture is the Starett scale 
on the front with the venier below it.

This mill came out of the AEC and spent its first life cutting graphite. 
After many hours of cleaning this machine getting all of the black off 
of it I found the ways almost perfect and the paint on it is the original 
as far as I can determine. This is a 3-phase machine with a 2-speed motor 
with reverse.  I think it to be original as the spindle speed chart shows 
speeds for both speeds of the motor.
